The insurance products and services are offered by Oriental Insurance LLC, a subsidiary of Oriental Bank. Oriental Insurance LLC is a General Agency and Authorized Representative authorized by the Office of the Commissioner of Insurance of Puerto Rico to offer life, variable life, variable annuities, disability, property & casualty, title and health insurance products. The insurance products and annuities are not a deposit or other obligation of or guaranteed by Oriental Bank or an affiliate of Oriental Bank. The insurance product or annuity is not insured by the Federal Deposit Insurance Corporation (FDIC) or any other agency of the United States, Oriental Bank, or an affiliate of Oriental Bank. For insurance products or annuities (if applicable), that involves an investment risk, there is investment risk associated with the product, including the possible decrease of value and the loss of principal.

Oriental Bank and Oriental Insurance LLC requires to obtain, verify, and record identifying information that identifies anyone who apply for or acquires an insurance product. Therefore, we will ask for your name, address, date of birth, social security number and other information that will allow us to identify you. We will also ask you to show us your driver's license or other identification documents.

In addition, when dealing with a covered legal entity (formed under the law of a foreign country and that have registered to do business in United States), we must ask you for identifying information (name, address, date of birth, social security number, % ownership, as well as identification documents) for each individual who has final ownership (25% or more) and for an individual who has significant administrative control of the legal entity. In the case of a legal entity, you will be required to provide the appropriate documentation (Certification of Final Beneficiaries) and certify that this information is true and accurate to the best of your knowledge.
